Choose between two things
Entweder ... oder means either ... or. Put the two parts before matching alternatives, such as two noun phrases. The examples describe invented options, not rules for real contracts.
Wir wählen entweder den Basistarif oder den Komforttarif.
We choose either the basic plan or the comfort plan.
Eva braucht entweder eine Bestätigung oder eine neue Kopie.
Eva needs either a confirmation or a new copy.
Der Anbieter schickt entweder einen Brief oder eine E-Mail.
The provider sends either a letter or an email.

Der Vertrag means contract, die Versicherung insurance and der Tarif plan or rate. Keep the alternatives parallel so the listener can see exactly what the choice is.

Choose between two actions
Entweder ... oder can connect two infinitives after a shared modal verb. Both actions then belong to the same subject. Kündigen means to cancel a contract; verlängern means to extend it.
Wir können den Vertrag entweder verlängern oder kündigen.
We can either extend or cancel the contract.
Eva möchte entweder anrufen oder eine E-Mail schreiben.
Eva would like either to call or to write an email.
Ben will den Beitrag entweder monatlich oder jährlich zahlen.
Ben wants to pay the premium either monthly or annually.
Ihr könnt das Formular entweder drucken oder speichern.
You can either print or save the form.
In Wir können den Vertrag entweder verlängern oder kündigen, können applies to both alternatives. The two infinitives keep the same grammatical form.

Two full clauses
For two full clauses, Entweder can come first, followed by the finite verb and subject: Entweder rufe ich an, oder ich schreibe eine E-Mail. Oder itself does not occupy a clause position.
Entweder rufe ich an, oder ich schreibe eine E-Mail.
Either I call, or I write an email.
Entweder prüft Eva den Vertrag, oder Ben liest ihn.
Either Eva checks the contract, or Ben reads it.
Entweder wir verlängern den Vertrag, oder wir kündigen ihn.
Either we extend the contract, or we cancel it.
Entweder kommt die Bestätigung heute, oder wir fragen morgen nach.
Either the confirmation comes today, or we ask again tomorrow.
Keep each clause complete. A sentence about a deadline in these examples describes only the fictional conversation; actual deadlines must come from the real agreement.
