Acting as if: als ob with Konjunktiv II
Als ob and als wenn introduce a comparison with something that is not true or at least doubtful. They mean the same and send the conjugated verb to the end of the clause. The verb is usually in Konjunktiv II: Er tut so, als ob er nichts wüsste.

Der Radfahrer tut so, als ob er von nichts wüsste.
The cyclist acts as if he knew nothing about it.
Die Läuferin wirkt, als ob sie gar nicht müde wäre.
The runner seems as if she were not tired at all.
Der Verband reagiert, als ob es kein Problem gäbe.
The federation reacts as if there were no problem.
Die Mannschaft spielt, als wenn sie doppelt so viel Kraft hätte.
The team plays as if it had twice as much strength.
Typical main clauses use so tun, wirken, aussehen or klingen. In formal texts you may also meet Konjunktiv I, such as als ob er nichts wisse. Der Radfahrer means the cyclist and der Verband the federation.

As if it had happened
To compare with something that supposedly happened earlier, put hätte or wäre plus a participle at the end of the clause: als ob nichts passiert wäre. Choose hätte or wäre exactly as you would in the Perfekt.
Der Sprinter jubelt, als ob er fair gewonnen hätte.
The sprinter celebrates as if he had won fairly.
Die Funktionäre tun so, als ob der Skandal nie passiert wäre.
The officials act as if the scandal had never happened.
Der Arzt spricht, als wenn er die Mittel nie verschrieben hätte.
The doctor talks as if he had never prescribed the substances.
Nach der positiven Probe schweigt der Verein, als wenn nichts geschehen wäre.
After the positive sample, the club stays silent as if nothing had happened.
Passieren and geschehen take wäre, while gewinnen and verschreiben take hätte. Der Skandal means the scandal, die Probe the sample and die Mittel the substances.

Als on its own: the verb comes next
In a shorter, slightly more written style, als can stand alone. Then the conjugated verb follows als directly, and the subject comes after it: als hätte er nie verbotene Mittel genommen. The meaning is the same as with als ob.
Der Athlet tut so, als hätte er nie verbotene Mittel genommen.
The athlete acts as if he had never taken banned substances.
Die Sperre wirkt, als wäre sie nur eine Formalität.
The ban seems as if it were only a formality.
Nach dem Dopingtest jubeln die Zuschauer, als wäre nichts geschehen.
After the doping test, the spectators cheer as if nothing had happened.
Die Funktionärin klingt, als hätte sie den Bericht nie gelesen.
The official sounds as if she had never read the report.
Never combine ob with this word order: als ob hätte er is wrong. Verbotene Mittel means banned substances, der Dopingtest the doping test and die Sperre the ban.
